FINANCIAL POLICY

We do everything possible to hold down the rising costs of medical care. The following is a summary of our financial policy.

1. AUTHORIZATION PROCESSING FEE: Our office requires a payment of $250.00 prior to your first appointment. This fee helps cover our costs of obtaining insurance authorizations, conducting the support group meetings and social functions. This fee is separate and will not be billed to your insurance company. We require a $50.00 deposit to hold your appointment for orientation and the remaining $200.00 is due at your first appointment. If you decide to cancel your orientation, with 2 days notice , the $50.00 deposit will be refunded to you. If you decide to cancel your first appointment, with 3 days notice, the $200.00 will be refunded. - This fee will not be refunded under most other conditions including, but not limited to, any of the following conditions: If you don't attend your orientation or your appointment and failed to give notice, if you cancel your orientation or appointment with less than the required amount of time or if you start the process and decide after your first appointment that you don't wish to continue.

2. INSURANCE: We bill participating insurance companies as a courtesy to you. You are expected to pay your deductible and copay at the time of service. You are responsible for all charges. We will discuss payment arrangements to satisfy the pre-surgery deposit at your New Patient Appointment. This deposit will vary with each individual Insurance Company's policies. - We will bill your secondary insurance. - We accept cash, personal checks, money orders, Visa and MasterCard. There is a $25.00 service fee for returned checks. If you need assistance or have questions, we will be happy to help you. Please contact the billing office between 8:00 a.m. and 5:00 p.m., Monday through Friday.

3. EXCLUSIONS: Some insurance companies specifically excludes weight loss surgery or treatment for obesity-related services. Unfortunately at this time, we are unable to bill insurance companies that state these exclusions. Patients that have this type of coverage will need to pay their bill in full prior to surgery.

4. PAYMENT PLANS: A monthly payment plan may be established through our billing office. If you would like more information on how to set up a payment plan, please contact our billing office.
